Dr. Smith will continue to provide injectables, facial cosmetic and plastic reconstructive services and surgery to include functional blepharoplasties through Skin Secrets Facial Plastic Surgery and Medical Spa, 580-536-5192, that will continue to occupy the current location.
Patient records will be available at the Allergy Ear Nose and Throat Institute until 12/18/25. After this date, a Custodian of The Records will be posted on our website: allergyentlawton.com. A paper copy of the patient’s last visit, that should contain all pertinent ENT information, will be available on request free of charge to patient to pick up. Also, available free of charge, is e-faxing the patient’s medical record to another provider’s office. Paper copies of patient charts will be available for $1.00 for first page and then $.50 per page for rest of the record, to be picked up.
